By submitting this form, I hereby give my permission for the individual under 18 years old (child) listed above to participate in church events/ activities while under adult supervision. I assume all liability for my child's participation in church activities/ events, and any injury that may result during church events/ activities. I understand that by signing this form, I release and hold harmless Live Oaks Bible Church, church and/ or event staff, employees, and/ or volunteers, from any liability, past or future, fully and completely for any injury suffered by my child listed above, that occurs during church activities/ events and/ or church transportation which may be in church, rented or personal vehicles. This Waiver and Release is valid beginning on the date below, and continues until my child turns 18 years old, unless revoked in writing by myself or another parent or guardian of the child listed above.
